Very rare, originally Yorkshire dialect but revived into wider usage because, well, it's such a fun word -- and sounds naughtier than it actually is. Origin is obscure, but the tit- almost certainly comes from the same root meaning a speck that gave the titmouse its name, and there's some hints that it actually started as an alternate name for a titmouse -- but regardless, I'd rather a tittynope of dessert than to delve deeper into that.
