That last is the original meaning from the 1560s, specifically any anatomical break or opening -- making this an example of medical jargon that went mainstream. As medical jargon, it is of course from Latin, from hiā(re) to gape/open + -tus, a suffix of verbal action.
And with exactly a year hiatus, life has settled down enough that I think I can resume this feature. I don't promise that I'll manage every weekday, but I'm going to try.
