The surgical removal is generally for medical reasons, such as cancer. When we're talking about castration, it's a highfalutin' synonym. Of course testis, as the medical term for a testicle, is a bit elevated in tone itself. Coined around 1894 from Ancient Greek roots órchis, testicle + ektomía, a cutting out of (from ektémnō, to cut out, from ek, out + témnō, to cut).
