According to this post, coined in 1652 from Latin quomodocunque, in whatever way, from quo modo, in what way?, which is taken from the OED. Aside from that venerable institution, this doesn't show up in dictionaries so much as lists of obscure words, but Google also finds the occasional blog post using it to make a point.
