Both literal and as an all-purpose rude expletive. Not at all used in America: when I met it in a Discworld novel, I wasn't sure it wasn't invented slang till I saw it elsewhere. Current since the 1950s, origin unknown but one leading contender is alteration of gonads, much like the Americanism nads.
