Related by blood, literally. One's degree of consanguinity with another person often determines whether marriage or sexual relations are allowed. Adopted (ha!) in 1602 by Shakespeare in Twelfth Night from Latin consanguineus, from con-, together + sanguineus, of or pertaining to blood (from sanguis, blood).
